4. Ordering & Prescription Requirements
- All Continuous Glucose Monitoring (CGM) devices require a prescription from your healthcare provider.
- You agree to provide accurate and complete personal, medical, and insurance information.
5. Medicare and Insurance Billing
- We bill Medicare and other insurers for covered devices and services.
- You are responsible for any co-pays, deductibles, and charges for non-covered items.
- If Medicare or your insurer denies coverage, you are responsible for the full cost of the item.
6. Shipping and Returns
- Shipping is available within the United States only.
- Due to the nature of medical devices, returns are only accepted for items that are unused, in original packaging, and in compliance with Medicare regulations.
7. Privacy and HIPAA
Your health information is protected by HIPAA. Please review our Privacy Policy and Notice of Privacy Practices for details on how we protect your information.
8. Limitation of Liability
BoxedRX is not responsible for indirect, incidental, or consequential damages arising from your use of our services. Our total liability shall not exceed the amount you paid for the product or service.
